Does anyone on the list have a scantron form used for scanning in scores from Nationals a few years ago.  Working on a project for IMAC scoring and I am investigating using optical mark recognition software.  I am curious to see the layout of the form that was used at the Nats.

If you do, please e-mail me off the list and we can figure out how to get me a copy of the form.
